Central Oregon Coordinated Care Organization (CCO)

Central Oregon CCO Quality Incentive Measures (QIM) Proposal: The purpose of this program is to deploy QIM dollars received by the Central Oregon CCO in 2015 based on QIM performance in 2014. The CCO received $3.27M, and approximately $2.9M remains to be deployed for best and highest use in Central Oregon.

Application Deadline: N/A. Applications are being accepted on a rolling basis until all funds have been dispersed. Funding decisions are made on an ongoing basis as proposals are received.

PacificSource Community Health Excellence Program 

Through the Community Health Excellence (CHE) program, PacificSource makes financial and other contributions toward initiatives that are independently evaluated as having a significant impact for patients, regardless of their insurance status.

Application Deadline: 7/30/16. Funding decisions are made in Oct 2016.

Central Oregon Health Council (COHC)

COHC is excited to partner with the communities we all live, play, and work in. The Regional Health Improvement Plan (RHIP) is for the benefit of all Oregonians that call Central Oregon home; Crook, Deschutes, Jefferson and Northern Klamath Counties. Proposed use of funds must be aligned with the RHIP and Triple Aim. Proposals may be multi-year projects.

Application Deadline: N/A. Applications are being accepted on a rolling basis until all funds have been dispersed. Submitted proposals are evaluated, and funding decisions are made monthly.
